STUDENT REGISTRATION DOCUMENTS & FORMS

The parent/guardian must complete the Student Registration Form and upload the 6 documents to the Online Registration Application.

REQUIRED DOCUMENTS

To register, you must have the following documentation:

1. Birth Date Verification

2. Two Proofs of Residency including one of the following:

  • non-contingent sales contract
  • current lease/rental agreement
  • most recent income tax return or IRS Form 1099
  • current paycheck stub
  • current warranty or quit claim deed
  • current home purchase agreement
  • current homeowner's insurance policy

AND one of the following:

  • current gas bill (within last 30 days)
  • current water bill (within last 30 days)
  • current electric bill (within last 30 days)
  • current cable/internet bill with service address (within last 30 days)

Students who are non-residents will be required to submit one proof of residency from the list in order to verify the mailing address for the student(s).

3. IF PROOF OF RESIDENCE is in a name other than the parent or legal guardian, a notarized school residency affidavit form must be completed and signed by both parties at the Registration Center. The homeowner will need to also bring a copy of a recent bill in the homeowner’s name.

4. GEORGIA CERTIFICATE OF IMMUNIZATION FORM #3231- Students must have immunization records from other countries or states transferred to Georgia form #3231. Forms must be marked complete for school attendance. Expired Forms will not be accepted.

5. GEORGIA VISION, HEARING, DENTAL AND NUTRITION SCREENING FORM #3300 - Services for both forms (#3231 and #3300) are available from the Hall County Health Department, 1290 Athens Street, 770-531- 5600.

6. PARENT OR GUARDIAN – must be present to enroll a student(s) and must present photo identification.

7. GUARDIAN must present a LEGAL GUARDIANSHIP DOCUMENT.

8. SOCIAL SECURITY CARD OR SIGNED WAIVER FORM.

9. WITHDRAWAL FORM, LAST REPORT CARD or RECORDS FROM PREVIOUS SCHOOL – Official high school transcript must be provided for high school students (grades 9-12) if he/she wants to transfer credits.

10. McKINNEY-VENTO - Families/Students who meet the federal definition of Mckinney-Vento (homeless/in transition) may be enrolled without the normal documents required for school enrollment

TRANSFER INFORMATION
Please see the  Gainesville City School System School Attendance Zone Map to find schools of assignment for addresses within the Gainesville City School District. 

Most students attend the school in the established attendance zone where they reside, unless a transfer request [Choice Enrollment] is granted in compliance with federal and state law; State Board of Education rules; and Gainesville City School District rules, policies, and procedures. Types of approved transfers include acceptance to a program of choice; assignment of children of employees; school choice; and out-of-district tuition) enrollments.

In-District Transfer Request (Choice Enrollment)

Choice Enrollment is available each year during February for the upcoming year. The application dates are posted on the GCSS website in January of each year. 

Family Moves

Currently enrolled students who move to a new residency address that is outside of the Gainesville City School District and who wish to remain in their current school may apply to be accepted into the Non-Resident Tuition program. The steps are outlined below:

  1. Please change your address through your Parent Portal. 
  2. Complete the Non-resident Tuition Application Process within 5 days of move
  3. If the Tuition Application is approved, parent/guardian must pay pro-rated tuition and student may remain enrolled at current school
